OHIO COUNTY SCHOOLS VOLUNTEER APPLICATION

Submit to the Principal at least 2 weeks prior to the event you are volunteering.  The Ohio County School Board will have to approve you before you are permitted to volunteer.  The best thing to do would be to complete at the beginning of the school year.  You MUST submit a new application EVERY year.

V.I.P. COMMITTEE VOLUNTEER FORM

Submit to the school office or turn in at the first VIP meeting following the Middle School Open House on September 15th.  Please include your e-mail address so we can contact you easily.  You can also contact any board member at any time during the year.

HAPPY AD REQUEST FORM

Submit with payment to the school office or to your student athlete’s coach at least one week prior to the first home game.  Photos and/or ads can be e-mailed to: warwoodvip@yahoo.com.
